The purpose of this assessment booklet is to document the students’ formative processes to make informed pedagogical decisions. Let’s agree grades do not speak by themselves, so it is necessary bring them to life.

With this assessment booklet, elementary or high school teachers could register qualitatively and objectively what students do when they are learning in order to complement formal assessments. Moreover, if this resource is applied systematically, educators could make evidence-based pedagogical decisions and substantiate important decisions for future assessments.

The booklet is organized into five interlocking sheets by student:

Sheet 1: This section is to fill in student personal information and register summative grades.
Sheet 2: This section includes two checklists to assess basic classroom rules.
Sheet 3: This section includes two rating scales to assess classroom interaction.
Sheet 4: This section includes two rating scales to assess work habits.
Sheet 5: This section includes an analytical rubric that synthetizes the previous 3 assessment tools before drawing conclusions.
